Does The Star Rating System Appropriately Measure Medication Adherence?

Med-Adherence-1

Medication adherence is a crucial component of the Medicare Star Ratings system, significantly impacting the ratings that determine reimbursements and incentives for health plans. The Centers for Medicare & Medicaid Services (CMS) have made medication adherence for conditions like diabetes, hypertension, and high cholesterol a triple-weighted measure, emphasizing its importance. However, the current system has several flaws that make it outdated and less effective in improving patient outcomes.

A recent article in HealthAffairs highlighted the fact that patients are often financially, and clinically, better off paying cash through various low cost generic programs, versus following their prescription benefit and it’s drug formulary:

“…the reimbursement landscape has changed dramatically since the measures were first introduced. Numerous large retail pharmacies have implemented low-cost generic programs that reduce the cash price for a prescription below the copayments required by Part D plans. As a result, many seniors choose to pay cash rather than file an insurance claim, reducing the accuracy of these measures.

Clinicians are now in the position of knowing that what is best for the patient may be at odds with performing well on the measure. Encouraging a patient to try samples or use lower-cost cash purchase options; advising a patient to try less frequent dosing; or encouraging a patient to resume a medication after a long period of non-use may all risk taking a hit on measure performance.”

Additionally, the Star Rating System often falls short across multiple other dimensions. The program uses a one-size-fits-all approach to measure adherence, which does not account for the diverse needs of different patient populations. This lack of personalization can lead to inaccurate assessments of a patient’s adherence and overall health (Louisiana Health Connect) (RxAnte.com). There is also an inadequate consideration of social determinants of health. Factors such as socioeconomic status, education level, and access to healthcare services are not adequately factored into the current adherence measures. These social determinants can significantly impact a patient’s ability to adhere to their medication regimen (Louisiana Health Connect).

RxLink offers a modern solution to these outdated measures by providing a comprehensive platform that identifies the best affordability programs for patients based on their prescribed medication and preferred pharmacy, regardless of their insurance plan. Here’s how RxLink can enhance medication adherence and simplify the process for both patients and providers:

  • Personalized Discount Programs: RxLink analyzes a patient’s specific medication needs and pharmacy preferences to find the best discount programs. This personalized approach ensures that patients receive the most cost-effective options, reducing financial barriers to medication adherence.
  • Insurance Independence: By focusing on the best available discounts irrespective of the patient’s insurance plan, RxLink ensures that all patients have access to affordable medication. This can be particularly beneficial for those with high out-of-pocket costs or those who are underinsured.
  • Ease of Use for Patients: The platform is user-friendly, making it easy for patients to find and apply discounts. This reduces the burden on patients to navigate complex pricing structures and insurance details, enhancing their ability to stay adherent to their prescribed regimens.
  • Reduced Provider Workload: RxLink streamlines the process of finding medication discounts, removing the administrative burden from healthcare providers. This allows providers to focus more on patient care rather than administrative tasks, ultimately improving the quality of care and patient outcomes.
  • Utilization Data Reporting: In order to satisfy health plan compliance reporting as it exists today, RxLink can provide comprehensive analytics on the utilization of all discount programs by health plan members, even when used off formulary.
